    An exploratory randomised trial of a simple, brief psychological intervention to reduce subsequent suicidal ideation and behaviour in patients admitted to hospital for self-harm.

    Background Implementation intentions link triggers for self-harm with coping skills and appear to create an automatic tendency to invoke coping responses when faced with a triggering situation. Aims To test the effectiveness of implementation intentions in reducing suicidal ideation and behaviour in a high-risk group. Method Two hundred and twenty-six patients who had self-harmed were randomised to: (a) forming implementation intentions with a ‘volitional help sheet’; (b) self-generating implementation intentions without help; or (c) thinking about triggers and coping, but not forming implementation intentions. We measured self-reported suicidal ideation and behaviour, threats of suicide and likelihood of future suicide attempt at baseline and then again at the 3-month follow-up. Results All suicide-related outcome measures were significantly lower at follow-up among patients forming implementation intentions compared with those in the control condition (ds>0.35). The volitional help sheet resulted in fewer suicide threats (d = 0.59) and lowered the likelihood of future suicide attempts (d = 0.29) compared with patients who self-generated implementation intentions.     Development of Technologies for Separation and Functional Improvement of Individual Milk Protein Fractions

    End of Project ReportMilk proteins can be hydrolysed (i.e. fragmented) using proteolytic enzymes to give enhanced functional and nutritional properties. There is an increasing demand for hydrolysed protein ingredients with specific properties for nutrition of individuals with specialised dietary requirements including infants, the critically ill, the immuno-compromised and athletes. Such hydrolysed proteins can be specifically designed to provide distinctive tailor-made solutions to meet customer needs in these areas. This project explored the technologies for the production of two types of hydrolysates i.e. acid-soluble and glutamine-rich. Acid-soluble protein hydrolysates have potential in the fortification of acidic beverages, including soft drinks. Glutamine-rich hydrolysates are suggested as an optimal glutamine source for administration during periods of stress, such as recovery from strenuous exercise, or from surgery. Casein was selected as the protein for development of acid-soluble product and cereal protein for the glutamine-rich product. The main conclusions were as follows: A number of protein hydrolysate products with value added properties and the processes required for their manufacture have been developed and are available for uptake by the food industry. Laboratory investigations identified conditions for the generation of two casein hydrolysates with desirable functional properties. Scale-up conditions for the manufacture of these hydrolysates in the pilot plant were successfully developed. Both hydrolystates were 100% soluble at pH 4.6, exhibited clarity in solution at low pH in clear soft drinks and in caramelised beverages and were stable in solution over a wide temperature range (from 4 to 30ºC) for extended periods. Solutions containing these hydrolysates exhibited no foaming properties and had acceptable sensory properties, being considered as weakly bitter compared to unsupplemented solutions. These performance characteristics make the acid-soluble hydrolysates useful supplements for caramelised beverages, such as colas, and clear soft drinks. Six glutamine-enriched peptide products were produced at laboratory scale using two commercially available enzyme preparations. These products had desirable characteristics such as increased levels of peptide bound glutamine, low free amino acid and free pyroglutamate levels. Pilot plant processes were developed for manufacture of the two glutamine-rich hydrolysates with most suitable compositional properties and these were fully characterised chemically.     Field Theory Entropy, the HH-theorem and the Renormalization Group

    We consider entropy and relative entropy in Field theory and establish relevant monotonicity properties with respect to the couplings. The relative entropy in a field theory with a hierarchy of renormalization group fixed points ranks the fixed points, the lowest relative entropy being assigned to the highest multicritical point. We argue that as a consequence of a generalized HH theorem Wilsonian RG flows induce an increase in entropy and propose the relative entropy as the natural quantity which increases from one fixed point to another in more than two dimensions.Comment: 25 pages, plain TeX (macros included), 6 ps figures.     Adult Fast-Mapping Memory Research Is Based on a Misinterpretation of Developmental-Word-Learning Data

    © The Author(s) 2019. Fast mapping is often used to refer to children’s remarkable ability to learn the meanings of new words with minimal exposure and in ambiguous contexts. It is one thing to claim that children are capable of learning words this way; it is another to claim that this ability relies on a specific fast-mapping neurocognitive mechanism that is critical for early word learning. Yet that claim has recently been made in adult memory research and used as a theoretical justification for research into an adult fast-mapping mechanism. In this review, we explain why the existence of such a mechanism in children is not supported by developmental research and explore the implications for adult fast-mapping data and research

    Cigarette Design Features in Low-, Middle-, and High-Income Countries

    Previous studies have shown that country income grouping is correlated with cigarette engineering. Cigarettes (=111 brands) were purchased during 2008–2010 from 11 low-, middle-, and high-income countries to assess physical dimensions and an array of cigarette design features. Mean ventilation varied significantly across low- (7.5%), middle- (15.3%), and high-income (26.2%) countries (≤0.001). Differences across income groups were also seen in cigarette length (=0.001), length of the tipping paper (=0.01), filter weight (=0.017), number of vent rows (=0.003), per-cigarette tobacco weight (=0.04), and paper porosity (=0.008). Stepwise linear regression showed ventilation and tobacco length as major predictors of ISO tar yields in low-income countries (=0.909, 0.047), while tipping paper (<0.001), filter length (<0.001), number of vent rows (=0.014), and per-cigarette weight (=0.015) were predictors of tar yields in middle-income countries. Ventilation (<0.001), number of vent rows (=0.009), per-cigarette weight (<0.001), and filter diameter (=0.004) predicted tar yields in high-income countries. Health officials must be cognizant of cigarette design issues to provide effective regulation of tobacco products

    The acceptability of nicotine containing products as alternatives to cigarettes: findings from two pilot studies

    <p>Abstract</p> <p>Background</p> <p>This study aimed to explore issues that might impact on the acceptability and feasibility of offering smokers nicotine containing products either to quit nicotine use altogether by using as a short term means of quitting cigarettes or as a longer term substitute.</p> <p>Method</p> <p>Two small pilot studies, one in the UK (n = 34) involving face to face contact and direct provision of the product, the other in Australia (n = 31) conducted remotely with products sent in the mail.</p> <p>Results</p> <p>Nicotine lozenges were the most popular products, but significant minorities liked a smokeless product more. Use stimulated interest in quitting, and although many failed to use all the products provided, most were interested in future use, more often to help quit than as a planned long-term substitute.</p> <p>Conclusions</p> <p>These studies indicate an untapped interest in the use of substitutes to reduce the harmfulness of smoking. Studies of this sort do not inhibit interest in quitting nicotine altogether, and may facilitate it. The greater the range of products on offer, the more smokers are likely to try a product to quit.</p

    Trends in Self-Harm in Kuala Lumpur, 2005-2011.

    Acts of self-harm are not routinely tracked in Malaysia. The present study investigates the prevalence of self-harm in Kuala Lumpur, Malaysia, over a 7-year period. The aims were to: (a) assess the prevalence of self-harm; (b) examine any changes over a period of 7 years, and (c) identify correlates of methods of self-harm. Data were extracted from the hospital records of Kuala Lumpur Hospital to review trends in self-harm between 2005 and 2011. There were 918 episodes of self-harm across the 7-year period, with a significant peak in 2007-2009. The average rate of self-harm (7.7 per 100,000 population per year) was similar or lower than the rate of suicide (6-8 or 8-13 per 100,000) suggesting that genuine cases of self-harm are often attributed to other causes. Nevertheless, over-representation of young people, women and Indians suggest areas in which resources to prevent self-harm might usefully be targeted. Estimating rates of self-harm are fraught with problems and further research is needed to understand the economic and cultural barriers around seeking treatment for self-harm, reporting self-harm and classifying self-harm

    Cigarette ignition propensity, smoking behavior, and toxicant exposure: A natural experiment in Canada

    <p>Abstract</p> <p>Background</p> <p>This study used a 'pre-post' research design to measure the impact of the Canadian reduced ignition propensity law on cigarette toxicity and smoking behavior among Canadian smokers.</p> <p>Method</p> <p>The study was conducted in Ontario, Canada over a ten-month period in 2005-2006, consisting of 4 laboratory visits (baseline N = 61, final N = 42). At Visit 1, questionnaire data and biospecimens were collected. During the following 24 hours, participants smoked 5 cigarettes <it>ad libitum </it>through a topography recording device and collected their cigarette butts. Visit 2 consisted of a questionnaire and smoking one cigarette to measure laboratory topography values. After ten months, these procedures were repeated.</p> <p>Results</p> <p>Generalized estimating equations, with law status (pre and post) as a fixed within-subject factor, were used to determine changes in behavior and biomarker exposure. Overall, there were no significant differences in smoking topography, breath carbon monoxide, and saliva cotinine pre-post law (<it>p</it>>0.1). However, analyses revealed a significant increase in the summed concentrations of hydroxyfluorene metabolites (N = 3),, and 1-hydroxypyrene in urine, with at notable increase in hydroxyphenanthrene metabolites (N = 3) (<it>p</it><sub>Σhydroxyfluorene </sub>= 0.013, 22% increase; <it>p<sub>1-hydroxypyrene </sub></it>= 0.018, 24% increase; <it>p</it><sub>Σhydroxyphenanthrene </sub>= 0.061, 17% increase).</p> <p>Conclusion</p> <p>While the results suggest no change in topography variables, data showed increases in exposure to three PAH biomarkers following reduced ignition propensity implementation in Canada. These findings suggest that human studies should be considered to evaluate policy impacts.</p
